Fat vs. Filler: Choosing the Best Solution for Facial Volumization

When it comes to restoring lost volume in the face, patients today have two popular options: fat transfer (also called fat grafting) and dermal fillers. Both can rejuvenate the face, soften hollow areas, and create youthful contours. But which is the smarter long-term investment? WHAT IS FAT TRANSFER?

Fat transfer and fat grafting are two terms used interchangeably to describe the process of removing a small amount of fat from one part of the body (commonly the abdomen or thighs) and carefully injecting it into areas of the face where volume is needed. The appeal is that you’re using your own tissue—nothing synthetic—which makes the results look and feel natural.


There’s also a related technique called fat transposition, often performed during lower eyelid surgery. Instead of removing fat pads beneath the eyes, the surgeon repositions or “transposes” them into hollow areas. This might be a better option for patients with large under-eye bags and dark circles caused by the excessive volume.


CANDIDATES FOR FAT TRANSFER VS. FAT TRANSPOSITION

Fat transfer candidates are generally patients seeking natural, long-lasting correction for hollow cheeks, temples, under-eyes, or jawline. It’s especially effective for those already undergoing another procedure, like lower blepharoplasty or facelift, since fat can be harvested at the same time.


Fat transposition is better suited for patients dealing with under-eye bags and dark shadows caused by fat shifting or volume loss. Rather than discarding the fat, Dr. Schwarcz repositions it for a smoother, more youthful transition from eyelid to cheek.


WHAT ABOUT DERMAL FILLERS?

Dermal fillers are an excellent option for patients who want facial rejuvenation without surgery. Some of the most effective filler options include:

  • Restylane: Trusted for years, Restylane products (like Restylane-L, Restylane eyelight, and Restylane Kysse) work well for under-eye hollowing and lip enhancement thanks to their smooth consistency.

Unlike fat, fillers are synthetic substances, and while they provide beautiful results, they’re temporary. Most last anywhere from 6 months to 2 years, depending on the type and treatment area.


FAT VS. FILLERS: LONGEVITY AND RESULTS

Fat transfer has the advantage of permanence—once the fat cells establish a blood supply, they can last indefinitely. However, not all transferred fat survives, so multiple sessions may be needed for optimal results. Fillers, on the other hand, offer immediate results with no downtime from surgery, but they require repeat treatments to maintain the effect.


To achieve facial volumization, Dr. Schwarcz almost always prefers to use a patient’s own fat when possible, given its longevity, natural appearance, and safety profile. That said, fillers can be the right choice for patients who are not ready for surgery, don’t have sufficient fat for harvesting, or want to “test-drive” volume restoration before committing to something more lasting.

Recovery for fat transfer involves mild swelling and bruising both at the donor and injection sites, with results stabilizing after a few months. Fillers, by contrast, involve minimal downtime—most patients can return to daily activities right away.
